My Documents

Other documents such as Treatment Plans, Discharge Plans, and some assessments can be signed off on through the Patient Portal.

A. Treatment Plans

Once a Treatment Plan is completed go to the signatures tab and there will be a button that says send to Client Portal for Sig. This will then show up on the My Documents Tab for the Patient to Sign off on.

B. Discharge Plans 

Once the Discharge Plan has been completed, you can also send the Discharge Plan to the Patient Portal for Signature.  At the end of the Discharge Plan, there is Send to Client Portal for Sig. This will also be  in their Patient Portal for Signature under My Documents once this is sent to the Portal.
